Request creation is token-gated on this deployment — the token is in the pinned message in the sandbox Slack (or arrive via its /demo?token=… link).

Request type Approval (yes/no) Form (multi-field) Multi-select choice

Form sends a 7-field launch checklist (text, email, number, URL, date, select, user) — enough fields that the Slack modal paginates. Multi-select choice sends a checkboxes card — the response is an array of every option picked.
